Changing Faces provides a community stabilization program catering to adults, children, and adolescents facing psychiatric crises. In moments of mental health challenges, an individual may struggle to attend to their own needs. If your loved one is in a situation where self-care is challenging, experiencing isolation from their community, or at risk of homelessness, hospitalization, or incarceration, our community stabilization service is readily available. To be eligible for the program, the individual in crisis should:
- Qualification criteria: Virginia Medicaid eligible, at high risk for psychiatric hospitalization, isolating themselves, unable to perform daily duties, and unable to recognize danger.
- Accepts various referrals, including self-referral, community, physician, family/friend, school, therapist/psychiatrist, and Department of Social Services.
- Unable to perform daily duties, resulting in putting their health and safety at risk
- Unable to recognize when they are putting themselves in danger
Our Community Stabilization program accepts a variety of referrals. In the best situations, this can be a self-referral. However, oftentimes, this is community referrals, physician referrals, family/friend referrals, school referrals, therapists/psychiatric referrals, and referrals from the Department of Social Services.
